Program Summary:

We will review how medical records are produced and how we as clinicians approach reading a medical record for legal cases. Hospital and clinic workforce who contribute to clinical document and their respective roles and authorities will be detailed with emphasis on how lawyers might identify potential experts. We will review how diagnosis codes are generated, how we put together summary narratives, and how to identify what information might be missing.
